Several findings are about Discord's own safety settings rather than about Defendr. Requiring two-factor authentication for moderation, a verification level of Medium or higher, the explicit media filter for all members, a safety alerts channel on Community servers and Discord's raid alerts all reduce what reaches your members before Defendr sees it.
Two more findings look at your server's own shape: dangerous permissions on @everyone — Administrator, Manage Server, Manage Roles, Manage Channels, Manage Webhooks, Mention Everyone, Ban and Kick — and temporary invites whose code contains uppercase letters, which are a common hijack target.
A mention-spam AutoMod rule is Discord's own first line against mass pings. Create it in Server Settings › AutoMod with a limit of 15 mentions and raid protection on.
